Skilled Nursing Facilities

Physiatry coverage built for skilled nursing facilities.

A PRC physician rounds twice per week inside your building, manages the clinical picture, and keeps families and social workers informed, so patients discharge safely and on time.

Bring physiatry in-house

Our primary goal is exceptional care and consistent clinical oversight.

Facility Presence

Rounding twice per week on each patient to evaluate and follow up on therapy and rehabilitation progress.

Discharge Planning

Evaluation of discharge planning and recommendations for safe discharges back into the community.

Communication

Frequent and early family, caregiver and social worker communication for the smoothest transition of care.

Clinical Management

Extension or Reduction

Changes to length of stay based on progress made and patient care needs, in collaboration with the facility.

What our physicians manage

Physiatrists ensure progression of overall function through rehabilitation.

Addressing any issue or concern that could hinder or limit progress with therapy, and the return to the patient's optimal state.

Long-Term Patients

Examine and evaluate for functional decline and the potential benefits of therapy, to help ensure a patient progresses back to baseline.

Clinical Documentation

Clear, organized and thorough. Supports need for therapy, RAC audits and critical documentation when meeting criteria for state and federal surveys.

Expert Support

Clinical expertise and educational support to the facility's care team, including administration, physicians, nursing, rehab team and staff.

Collaboration

Working with the specialists and the primary care team.

Specialist collaboration. Communicating directly with orthopedic, vascular and spinal surgeons to promote healing and carryover of recommendations, and evaluating for higher level of care.

Procedures and injections. Peripheral joint and trigger point injections to help reduce pain and enable increased benefit from therapy.

Primary care. We work closely with primary care physicians to ensure patients' clinical needs are met, and help nursing staff address acute conditions as they arise.

What changes

What consistent oversight changes.

Patients do significantly better with consistent oversight utilizing goals set by therapy, with a focus on motivational strategies.

Nursing staff have a physician to address any acute conditions or concerns that arise.

The rehab team has a partner to ensure productive rehabilitation efforts and improve overall outcomes.

Coverage comes at no cost to the facility.
