Our primary goal is to extend exceptional care and consistent clinical oversight to patients in skilled nursing facilities. Physiatrists (PM&R physicians) with Pacific Rehab Consultants ensure progression of overall function thru rehabilitation, addressing any issue or concern that could hinder or limit progress with therapy and return to patient’s optimal state.
Rounding twice per week on each patient to to evaluate and follow up on therapy & rehabilitation progress.
Evaluation of discharge planning and making recommendations for safe discharges back into the community.
Frequent and early family, caregiver & social worker communication for the smoothest transition of care plus safe patient discharges and best outcomes.
Making recommendations for rehabilitation, acute medical issues, general health, pain management, DVT prevention, pressure ulcer prevention and management.
Evaluate need for changes to length of stay based on progress made and patient care needs in collaboration with the facility.
Examine and evaluate for functional decline and potential benefits of therapy to help ensure a patient progresses back to baseline.
Clear, organized and thorough; supports need for therapy, RAC audits and critical documentation when meeting criteria for state/ federal surveys.
Clinical expertise and educational support to the facility’s care team including administration, physicians, nursing, rehab team and staff.
Communicating directly with orthopedic, vascular and spinal surgeons to promote healing and carryover of recommendations. Evaluating for higher level of care.
Performing procedures such as peripheral joint and trigger point injections to help reduce pain and enable increased benefit from therapy.
It is our experience that patients do significantly better with consistent oversight utilizing goals set by therapy with a focus on motivational strategies. We work closely with primary care physicians to ensure patient’s clinical needs are met. We help nursing staff to address any acute conditions or concerns that arise and collaborate with the rehab team to ensure productive rehabilitation efforts and improve overall outcomes.
